RN Case Manager - SeniorBridge (Melbourne, FL)

Description SeniorBridge Care Management Program (SBCM) RN - Full-Time Needed ! SBCM is seeking passionate, case management professionals and leaders dedicated to delivering the highest quality in-home care management programs to complex chronically ill patients enrolled in large health plans and managed care programs. Requirements A minimum of two years in Nursing, experience in a health plan-based or hospital-based case management business unit plus at least three years experience in home care, skilled nursing facility or disease management Extensive clinical experience in one or more of the following disciplines or settings : Critical Care, Geriatrics, Med/Surg, Home care or another community-based or public health setting. Provide day to day SBCM Care Management services through out the Brevard County area. Ensure in-home care delivery is in accordance with each client's contractual program requirements is high quality and complies with company -adopted evidence- based clinical practice guidelines Prepare clinical activity reports as needed and defined by each client. BSN preferred. In lieu of BSN, an associate degree with 2-3 years experience in eldercare is acceptable Certification in Care Management is preferred Valid license to practice as a Registered Professional Nurse in State of Florida Must be a road warrior (Valid Driver's License is required)
